What can patients expect from the new Family Practice Clinic in Mount Horeb?
The Upland Hills Health Mount Horeb Clinic will provide family services in an expanded, renovated and state-of-the-art facility with care for all stages of life. Upland Hills Health’s Mount Horeb clinic will go beyond family medicine with imaging, an OB/Gyn clinic, and other specialty services within both the clinic and Urgent Care.
What are the hours for the family practice clinic and Urgent Care?
The family practice clinic will operate Monday – Friday, 8 a.m. – 5 p.m. To make an appointment with a provider, call 608-437-8033. Urgent Care hours are Monday – Friday 3 p.m. – 9 a.m., and weekends 8 a.m. – 4 p.m. Call the Urgent Care at 608-437-9003.
Who staffs the Urgent Care?
The Urgent Care is staffed by Madison Emergency Physicians, the same group of physicians and providers that staff SSM Health St. Mary’s Emergency Room and Upland Hills Health Emergency Room in Dodgeville.
Primary care physicians at the family practice clinic are the same experienced providers you’ll find at Upland Hills Health Hospital in Dodgeville. Many are Mount Horeb residents themselves, including Dr. Joseph Berg.
What insurances does the new clinic accept?
Upland Hills Health accepts a wide variety of insurance plans. Please contact your insurance company to see if Upland Hills Health and the provider you wish to see are considered in-network with your specific plan. You can find the contact information on the back of your insurance card. It is possible some insurance companies will limit the services we are allowed to provide.
