  • Think about your health care values or goals
  • Talk about your choices with your doctor and loved ones
  • Make a written plan, called an advance directive. This will include you choosing the person you want to make health care decisions when you’re unable to communicate with the doctor.

Wisconsin law requires this designation be in writing.
