Most people say it’s important to talk with your family about their future medical care and to put it in writing, yet only a few people do.
At Upland Hills Health we’re here to help you take the next steps to complete a written plan.
We’ll help you:
- Think about your health care values or goals
- Talk about your choices with your doctor and loved ones
- Make a written plan, called an advance directive. This will include you choosing the person you want to make health care decisions when you’re unable to communicate with the doctor.
Wisconsin law requires this designation be in writing.
