Q: Is the cost per day or per month for living at Upland Hills Nursing & Rehab Center?

A: The cost for living is per day. For current rates, contact our Social Services staff.

Q: How do I secure my place at Upland Hills Health?

A: For rehabilitation stays, your hospital discharge planner helps coordinate the transition to our short-term stay rehabilitation wing. For long-term care placement, rooms become available less frequently, as we are a residential facility. If you are interested in discussing short or long-term placement, contact us

Q: How often will my doctor see me?

A: Your doctor will see you once a month for the first three months. After that, your doctor will see you every 60 days, depending on your health status.

Q: What personal items can I bring from home?

A: Rehab residents typically bring comfortable clothes and shoes, pajamas, slippers and their preferred toiletries, if they choose not to use the toiletries we provide. Long-term care residents are allowed to bring larger personal items such as chairs and tables, as long as they fit comfortably in the room. We do have basement storage for residents’ seasonal clothes or decorations.

Q: Are there set visiting hours?

A: No, we don’t have set visiting hours because we know how much our residents appreciate visits from friends and family. The only time visiting is restricted is if there’s an outbreak of flu or widespread communicable disease in the community.

Q: Can I bring my vehicle?

A: Yes, however a personal vehicle is not necessary for transportation. The Hodan Center, Countryview, County Taxi and Aging & Disability Resource Center are available for transports. For more information, please contact our front desk at (608) 930-7600.

Q: What is your pet policy?

A: Dogs and cats are allowed to visit the Nursing & Rehab Center’s residents, and all other animals may be approved on a case-by-case basis. The Activity Director must be provided with current copies of immunization records and the floor nurse should be aware of the planned visit to ensure the timing is appropriate.

Q: What is a resident trust fund?

A: A resident trust fund is money held by a skilled nursing facility for a resident of the facility.

Q: Can my family and friends bring me food and snacks?

A: Yes, family and friends are allowed to bring in food and snacks. However, the food/snacks must be brought to a CNA first so that it can be labeled with who it is for and the date. Other residents are not allowed to eat the food unless they sign a waiver.

Q: What is your smoking policy?

A: Upland Hills Nursing & Rehab Center is a tobacco-free facility. No smoking is permitted on campus grounds.
