Upland Hills Health Mount Horeb Clinic Announces Temporary Changes

The Upland Hills Health (UHH) Mount Horeb Family Practice and Therapy Clinics have experienced unprecedented growth over the past few years. To meet this increased demand, UHH will be remodeling and temporarily closing the Family Practice Medical Clinic in Mount Horeb. During this time, providers will see patients at the UHH medical clinic in Barneveld located at 103 Quail Ridge Drive. The Therapy and Wellness Center will continue to see patients on the second floor of the existing clinic.

If you have a medical or therapy appointment scheduled on or after April 1 and you have not received a call from a Upland Hills scheduler or therapist, please call the Mount Horeb Medical Clinic at 608-437-8033. You can call the Mount Horeb Therapy and Wellness Center at 608-930-7200 x2171 or the Dodgeville Therapy and Wellness Center at 608-930-7147 to confirm that you will be seen on the second floor of the Mount Horeb Clinic instead of on the first floor if you have not been contacted by a therapist.

The medical clinic expansion includes opening the only Urgent Care Clinic in Mount Horeb. The Urgent Care Clinic will be staffed by UHH’s experienced team of physicians and providers to increase your access to after-hours care. The remodeled medical clinic will reopen later this fall.

The Therapy and Wellness Center will be moving to 1883 Springdale Street, which is located near Miller & Sons Supermarket. It is expected to open in early fall, 2024.
